How to Create a Standard Referral

Learn how to efficiently create a standard referral in Lutra Health with this comprehensive guide.

1. Search for or register your patient

A referral has to be made from a patient record. If the patient has already been added on Lutra, then you can search for them. If not, then you can register them.

2. Start New Episode

Click Start Episode to begin documenting a new clinical episode for the patient.

3. Initiate New Referral

Click New Referral to create a referral for the patient.

4. Select referral condition

Select the required referral condition from the drop down list.

5. Select Recipient Field

Click Select a recipient to choose the referral destination.

6. Select Referral Type Standard

7. Select eye

Select which eye the referral relates to.

8. Select priority

Select the priority level for the referral from the list of available drop down options.

9. Start Referral Process

Click Start to initiate the referral process.

10. Provide Vision Assessment Measurements

Provide Vision Assessment measurements. A red asterisk indicates the mandatory fields.

11. Option to add any additional notes

You can optionally add any further comments or notes about the vision assessment, before proceeding to the next step.

12. Pathology details

The next steps enable any relevant pathology details to be provided for lid, corneal, lens and posterior segment.

13. Option to provide IOP and visual field results

Optionally, intraocular pressure and visual field results can be provided. Where this is not relevant, then this step can be skipped.

14. Complete form questionnaire

Complete the form questions. These allow you to capture information such as duration of symptoms, whether the patient requires an interpreter, or has any mobility considerations.

15. Upload Referral Files

The option is available to upload one or more files, for example any relevant images.

16. Upload and tag file

If any files are uploaded, then these need to be tagged with eye and file type details.

17. Add any additional assessment notes

Optionally, add any additional assessment notes in the text box. If required, there is also a field where you can add if you are referring to a specific clinician.

18. Review and submit referral

A summary page allows you to review the referral prior to submission. If changes are required, you can go back to the previous step to make these. When you click on 'Submit Referral' this completes the process and the referral is sent to the recipient organisation.

19. View Outgoing Referrals

If you wish to view referrals your organisation has submitted, you can view these by navigating to 'Referrals' and then filtering by 'Outgoing'. You can click into any record to see more detail.
