How to Create an Advice and Guidance Request

This tutorial guides you through creating an Advice & Guidance request within Lutra Health.

Go to app.lutrahealth.com

1. Click Start Episode

Click the Start Episode button to begin the process of creating an Advice & Guidance request.

2. Select Advice And Guidance

Click the Advice & Guidance option to open the form where you can create a new request.

3. Choose Condition Category

Select the relevant medical condition for the request.

4. Open Organisation Selector

Select the recipient organisation of the request.

5. Select Affected Eye

Indicate the affected eye for this medical condition.

6. Select Priority

Select the priority level for this Advice & Guidance request.

7. Choose Duration of Symptoms

Select the relevant duration of symptoms option.

8. Add Request Detail

Add request detail in the free text box.

9. Upload Files

Any relevant files can be uploaded. For each file, eye and file type needs to be tagged.

10. Enable Referral Conversion

By default, the recipient is authorised to convert this request to a referral if appropriate. This can be disabled, if required.

11. Create Advice And Guidance

Click Create Advice & Guidance to finalise and submit your request.

12. View Created Request Details

Click the created request entry to review the details
